Aiming for Higher Detection, Protection and Perfection of Treatment of Atrial Fibrillation (AF) Patients in Darlington CCG

The Oberoi Stroke Prevention & Case Finding Service was rolled out across all of NHS Darlington CCG’s 11 GP practices in June 2019.

Professor Ahmet Fuat, renowned for his expertise in Primary Care Cardiology, headed up its introduction and supported deployment at the practices which serve 108,273 patients in County Durham.

Public Health England (PHE) AF Targets

  • Reach 85% AF detection rate by 2029.
  • Reach a minimum of 90% anticoagulation rate in high risk AF patients by 2029.

What are the AF targets for Darlington CCG and its Practices?

  • Reach 82% AF detection rate by the end of the first year (June 2020).
  • Reach a minimum of 87% anticoagulation rate in high risk AF patients by the end of the first year (June 2020).

AF Detection rate of the CCG is 80.5%

  • 5/11 practices are above the CCG 82% detection target rate
  • 6/11 practices are below the CCG 82% detection target rate

AF Protection rate of the CCG is 91%

  • 8/11 practices are above the CCG 87% protection target rate
  • 3/11 practices are below the CCG 87% protection target rate

Perfect:

  • Out of the 1,419 patients prescribed DOACs across Darlington there are 91 that need to have a dose review.
